Complete the form, please make sure the form is completed in black ink and is clear and legible.

If you are sending us your forms by post we recommend that you send them by special delivery.

Payment
Payment to ‘Visa Only Service’ should be for the full amount. Visa fee, Handling Charge & Postage (if applicable)

Cheque
Please make cheques payable to ‘Visa Only Service’ Visa applications paid by cheque will only be sent to the embassy after the cheque has been processed by our bank.

Cash
Can be paid at our branch in Preston, Please do not send cash in the post.
